Use this form to fill out the type of linen you need.
Order only as many towels as one person can carry.
This will usually consist of 30 towels and 2 laundry bags.
and some face cloths if you need them.
We no longer have to fill in the account number.
Cross out the Ext. 24345 extension, and write in the following:
"please call the lab Ext. 24344 when order is ready"
Fax the form to the number 905-388-6577 (also listed on the form).
It will take 3 or 4 days for the linen to arrive.
A really nice lady named Irma will call you when the linen arrives
if you let her know where you can be reached.

When the new linen is ready, bundle up the dirty
linen and take it to the linen services at the hospital.
Linen services is
by hospital stores, Room number 1T4.
Do this before 3:30pm, they often close before that time.
